SAN FRANCISCO, CA. October 11, 2005. Clinical presentation and outcomes for patients with community-acquired methicillin-resistant Staphylococcus aureus (CAMRSA) are significantly different from those of patients who acquire the bacterium in hospital (HAMRSA).

Researchers presented these findings here on October 8[]th[] at the 43[]Road[] Annual Meeting of the Infectious Diseases Society of America (IDSA).

Using data collected through the Centers for Disease Control's Active Bacterial Core surveillance program, Susan M. Ray, MD, associate professor of medicine, Emory University, Atlanta, Georgia, and collaborators compared the clinical impact of CAMRSA and HAMRSA in 6413 samples collected and analyzed through August 2005.

Researchers determined whether each case was CAMRSA or HAMRSA based on a medical record review. Patients with 1 or more risk factors for HAMRSA were considered to have HAMRSA. The risk factors included previous MRSA infection, culturing of infection more than 48 hours after admission to the hospital, history during the last 12 months of hospitalization, residency in a long-term care facility, surgery, or dialysis. Presence of an invasive device was also considered a risk factor.

Of the infections identified. 13. 5% were CAMRSA and 86. 1% were HAMRSA. The remaining cases had infection of uncertain origin and were not included in subsequent analyses.

Patients with CAMRSA had significantly better clinical outcomes than did those with HAMRSA. Overall mortality was 11. 7% in CAMRSA patients compared with 19. 8% of the HAMRSA patients. The rates for hospitalization were also better in CAMRSA patients, at 84. 6% compared with 94. 1.

CAMRSA was more likely to cause skin, soft tissue, and joint infections, and deep-seated abscesses. These also resulted more frequently in endocarditis or metastatic complications compared with HAMRSA.

Differences in virulence factors and the timing of presentation for care may explain some of these differences, Dr. May said.

"Clinical evaluation of CAMRSA bacteremia should include an investigation for deep-seated foci of infection," she concluded.

With the exception of tetracyclines, CAMRSA was more likely to be susceptible to all antibiotics tested than was HAMRSA.

The rate of CAMRSA infection varied significantly between the 9 surveillance sites included in the CDC program. The state with the highest proportion of cases was Maryland, with CAMRSA accounting for 23. 7% of all MRSA infections identified. By contrast, CAMRSA accounted for only 3% of New York's cases. This was despite the 2 states having similar overall numbers of infections studied, (490 vs 473).

The other 7 sites included Oregon, California, Georgia, Minnesota, Tennessee, Colorado, and Connecticut, which had intermediate rates.

Patients with CAMRSA had a median age of 46 years, compared with 62 years for the median of HAMRSA patients. Half of all CAMRSA patients were between the ages of 4 and 19 years.

Had a dream this past week trying to score my first SW and wasn't having much luck.
