It's like GFE, it's subjective and as defined by the provider in their menu of services, so it varies among each provider. Mostly, it's about protection and the exchange of bodily fluids.
GFE PLUS may include. DFK, BBBJ, DATY, CFS, CIM. MSOG, etc.
It's just a way for them to justify higher rates and to charge more for "plus services." I can't wait for GFE premium or GFE extra to show up if it hasn't already.
