Now that you have a couple of ladies that can vouch for you you never should have to give your personal info again. I went a different route and became a regular at an agency. When I went to an ind prov that wanted a referral I gave her the contact at the agency. If I ever try to set an appt with a Provider that isn't comfortable with my referrals I will walk away. He had no clue why he was put on the list. He asked if I had any suggestions. Although I have no first hand experience in these matters, I referred him to other web sites that have more info. Later I tried to PM him to see how it was going, but his I D is not longer in USASG.
