And yes the intake forms are now required by the Fla Dept of Health that regulates the massage industry along with a lot of other new requirements. And you will usually get your temp taken with a forehead thermometer.

I think blue ray spa is new or doesn't have a Google my business page they are in orange park across the orange park medical hospital. They are ducted off in a corner so you have keep a eye out I have there phone number I'm ok with sharing. And so far I might be paying them more visits in the future.

I can almost guarantee that she's not legit. First of all, I've never heard of a girl from any of these sites being legit when she asks for a deposit (I fell for it once too). Second of all, her pics are way too good to be true for what's generally available around here (at least the ones that advertise online).
