I walked past 98 Mott earlier today to check out the area and I noticed the building had two entrances. This report is a couple months old, but the spa is still open and worthy of a visit. I hit this place up one evening when I was downtown taking in a movie. A couple of floors up on west 45th st. The 14th building (haven't been there in a while but last time it fit your requirements).

On west 46th st. On the 6th floor of either 48 or 46 (I get them confused).

Gents on the board prefer 98 mott but if you're coming from CT. I'd stay in midtown. Definitely take the train. Parking and traffic are nightmares. Besides, you can have a beer on the ride back and relish in the moment.
