I could never get clear answer on what the extra 40 for GFE Plus gets you at those agencies. Standard rate gets BBj, covered full service and shower. What gives?

These are just made up terms that vary from provider to provider. Agency to agency and even among hobbyists. Everyone has a different definition on these things. It's why I've given up on these terms, IMHO they're total BS.
That said, a lot of agencies or girls that use "GFE Plus" often also use it as a hint that they do greek.

Could care less about that so never paid the extra fee but was available everytime I went even though the girl knew I only paid the GFE rate.

As for Miso or other Asian agencies, I do not know.
