It wasn't to intrusive that I can remember, but everyone has they're own level of comfort of info they like to give up. I don't have an employer or SO, so giving out info didn't bother me.
Give them a call and see what they will require of you. That's better than second hand info.

I am glad it worked out for you as well as it did, but I personally would NEVER EVER (did I say ever LOL) give personal info to a provider. It could be Miss America herself and it just ainta happenin. I learned my lesson the hard way. Back in DC about ten years ago, I had a provider come to my room.
