The form lists all the tests that needs to be done, that's how they figure out how much blood to draw.

One caveat. Better check and see what the Law is in your state regarding anonymous testing, especially for HIV. In some states they are REQUIRED by law to verify who you are, using state issued ID before they will test you. So even IF you buy the services online, when you show up at LabCorp or Quest Diagnostics in such a state they will ask for ID. Oh sure, might find a phlebotomist who is new, unknowledgeable or lazy and they don't ask for ID on a prepaid test. But if you show ID, in that situation you end up filed in a State Database with the date of testing and results. Not so good if one is married or with a SO.

The privacy concern is that with the force of law (supeona, etc) nothing "on record" is truly private. You get in a nasty divorce proceeding and it is discovered you got STD checked a bunch of times. What happens when Apple, Google, FB or some business decides it is in the best interest to spy on your cell phone to protect the "greater good?" What happens when the STD check records of one of those testing sites gets hacked and published in some Ashley Madison style breach? Who knows what the next iteration is of less privacy or "outting" of people is?

If anyone doesn't get that, then they are open to the consequences.

State can be a "California wannabe" and still require I'd to get STD testing. My point is check the laws or regulations in your state before [I]assuming[/I]. There is a difference between so called "annonymous" and private and confidential.
