In my general experience, it is rare that any fees are paid to the Mamasan / greeter and are more usually paid to the attendant. You can pay once (house fee plus tip) or pay each separately — that's more a matter of your preference— at least for your first visit. And at least the house fee will be collected prior to any activities starting. Separate payments usually results in a pause in the action for the tip to be made. If you become a regular (and they know and trust you not to rip them off) the payment timings may differ. At locations outside of the city proper (where I am known) I have had a complete visit be performed and concluded before any payment was made.

If you pay this to the girl, she will take it and go pay Mamasan and come back. Gives time to get undressed and under the towel. She'll come back and start the rub.

Pay for services, "tip": This is where negotiating skills come in. Do this on "the flip". You pay upfront with an unknown place / girl leaves chances for misunderstanding later. There is no BBB to complain to when disputes happen.
