They also have to report incidence to CT Health Department for data purposes but will not provide any PII (Personally Identifiable Information) per HIPAA laws so your identify is not disclosed.

Notice the statute says SHALL report. In legalese, that means it isn't optional.

The form will have the patient's name. It also has a PARTNER NOTIFICATION SERVICES section. HIPAA just means the information isn't made public, it does NOT mean the health department doesn't know who you are.
