I've only had the 4 song VIP once, but I think it is actually a full 4 songs. As long as you don't get a VIP when they have specials on the regular floor, you will get more time. I've had the 3 song VIP twice before they changed the minimum to 4 and you could hear the songs in the background which are coming from the stage.

If you're gonna get a VIP at COI Rhino, get one with Skylar. You're guaranteed to bust a nut at no extra cost even if it is only a hand job, but she might do more if she really likes you or if you get her more than once.
