I swear I saw one of those forms on their site.
But I did send Pam's a text with a picture of my I'd and Business Card (I did cover some of the information) so I thought I would have everything they need. It got me in at OT and Wisteria. It helped at Wisteria that I was already a visitor of OT. I will keep trying.
