I've never done the SW thing and am completely unqualified to answer any of your questions. But obviously that's not stopping me from trying.
Also, I'm not sure how good an idea it is for you to ask for advice here, since USASG is a boys' club and intended mainly for the benefit of hobbyists. The great majority of us do view this as a hobby, not as an addiction or an immoral activity, so don't expect any insight into either of those possibilities.
Hobbyists' psychological motivations can as varied and nuanced as there are number of hobbyists, so I wouldn't necessarily take any general answers or consensus that you get here as gospel. But I have been several state here that it's the thrill of the hunt that makes them go for streetwalkers. He might be looking for a certain rush or thrill that he isn't getting within the marriage. That could be why he's gone from MP's to SW's; because he's looking to "AMP up" the thrill. If you can get him to really open up about this with you, maybe he can give you insight into his thinking and allow you to find a way to work with him through this. Although if you've both been to counseling before, I'm not sure how much new insight he'll have to offer.
But this isn't to say he doesn't love you or otherwise isn't putting as much effort as he can into the marriage. Men just aren't monogamous by nature. Monogamy is a sexual confinement put on men by society. And despite popular beliefs, most men who cheat still love and intend to stay with their partner. My personal suggestion is to consider an open relationship. I see a couple of others before me have already suggested the same thing. If that's not something you're willing consider, there's probably not much more guidance we can give you. Maybe that is true, in which case he is luckier than most. But I have never met a woman who would admit that a relationship breakdown was her fault. In her mind, it's *always* his fault. There's obviously another side to this story, and we are not hearing it, though many of us have our suspicions.
Many women that I have spoken to seem to believe that sex is some kind of favor that she graciously doles out to him. If she feels like it. And if he deserves it. That is what your tone suggests. Do you actually *want* him inside you? Do you *want* hair-pulling bed-shaking take-me-now-without-mercy-master sex, or are you just doing what you feel you have to to ensure his continued loyalty? Be honest. There is no wrong answer here.
Sorry if this seems harsh, but if you came looking for sympathy, you won't find it here. What you will find is a bunch of men who are, for one reason or another, incredibly frustrated and perhaps somewhat angry.
To answer your questions: STDs from receiving BBBJs are possible but rare, and mostly curable. IMO. There are very few "sex addicts" in the clinical sense, but a lot of men with unfulfilled high sex drives. Same goes for "nymphomaniacs", by the way, which are rarer still.
So where do you go from here? Are you willing to share him? An open marriage? The occasional threesome? The swinger lifestyle? Bringing a hooker home? Or are you willing to completely forgo sex with him for your own protection and let him seek it elsewhere, as long as he comes home to you?
The only way you can broach this is with love, understanding, forgiveness and a lack of judgement. Anything else will motivate him to cover up and give him a reason to lie.
