Using the burner phone does take some discipline and I normally keep it at the office at night and over the weekends. The only people who know who you are by doing it this way, is the register agent, the bank, and the IRS. It would take a court order to pull your name. As far as address used? You will use the register agent's. For $100 a year they scan all paper mail and email it to your new domain.

And the steps are not necessarily in the correct order; as I already had a domain.

You can't use a VPN especially when purchasing the membership as the payment processor will often block the transaction and then your SA account is suspended. Get on a local WIFI somewhere if that's your concern.
