In Detroit, it is only a few places that do it. Orlando, pretty much all. In Toronto, this was a few years ago, you have to shower yourself and go to the massage table when done. In the Seattle area, folks report paying between $200-$260 per hour at the AMPs, and this can include a table shower and CFS. Sadly, many AMPs here don't have table showers, though there are several (that do have table showers) on Highway 99 in Lynnwood with mostly older providers.
For this thread topic, I rarely pay above $200, can't remember the last time I did. I don't cry very hard if I don't get the full hour: anywhere between 45 minutes and 70 minutes, and I'm generally satisfied. I pay the provider directly.
My answer might not apply to JM specifically: VC reported great results. However, I've never known a first time poster who had a 5-star experience right out of the gate.
