No, of course they don't "Take the anus and make it a vagina": imagine the likelihood of post-op infection if they did this!

They just build a vagina (vaginoplasty) where the dick used to be, so a post-op TS is going to have two holes, an anus and an (artificial) vagina. I guess it's a pretty advanced procedure nowadays, as they can even enervate the built vagina (they can build a clitoris, for example). I don't think it self-lubricates yet, but I'm sure that's in the development pipeline.
