The below post does not answer the next question: what happens crossing back if you have prescription drugs you bought in TJ but do not have a prescription from a US physician?
I cannot provide an answer, perhaps others can.
On that topic:
I used to belong to a Tijuana-specific website. One posted question was, "What do you say at the border when you are asked, 'What are you bringing back?' ".

But you never know when it will rise again. If you see police hanging out on the street. It is a good idea to walk on the other side of the street. The police will hang out there and search people for no good reason. The police that act as security for the bars are not a problem however.

Posts: 66I've stayed at several of the hotels down there by myself, and have never had any problems. Well the only problem I've had was booking a room for a single person, and the front desk guy yelling at me when I tried to take a girl up to my room. I had to pay the front desk a "second person fee". I don't know if he was just trying to scare me, but it was only $10 so I paid it without arguing. You literally cannot turn around without seeing police somewhere on the streets around the HK club. The clubs (and gangs who have involvements with the clubs) do not want anything bad happening to the tourists in that area. The tourists bring in money, lots of money, and they don't want to have the tourists scared off. So that immediate area is about one of the (relatively) safest areas in TJ. That being said, you have to worry more about the Police in that area, than anything else. If you are acting drunk and stupid out in the street, they will try to shake you down. Or even worse, if they think you're looking for drugs, then you're going to have some really bad problems. If you stick to that area, you should be fine. Just don't wander off down one of the side streets, don't do anything stupid around the police, avoid anyone who appears to be selling drugs, and get away from anyone who says they can hook you up with "younger" or "fresher" girls.

Sex for sale is legal and the girls carry health cards. Few speak any English, and that is probably the biggest issue you will face. But I would have more problem walking in a SW area of Los Angeles that Tijuana, as far as safety is concerned.
