My experience was the same as yours twice, one time I had to fill out the form and got temp checked, the second visit a few weeks ago I did not.

I think it was the same lady though as the massage seamed the same. That's me. I turn my phone off or leave in the car, you got to be careful because google timeline tracks your routes and destinations.

I am married and wifey pays the cell phone bill and has access to calls made obviously. So I never call ahead of time, if they are busy I will get a time and try back later or just move on down the road.
