The people you are talking about that goes straight to the gate are ones with Global Entry or SENTRI. Both are programs run by Customs and Border Patrol that pre-vet you for entry into the US. Pretty painless process but heard it was pretty backed up now (for GE anyways). US citizens and permanent residents can get it.
