I hit the Thai massage places often, cause my back if foooked up pretty much all the time. I've yet to hear of any of the Thai places providing Extras now a days anywayz.
Probably the best Massage I've gotten was from here: http://honolulu.craigslist.org/oah/thp/3311374715.html
Was not the girl in the pic (was kinda hoping. He he) , can't remember her name, but she knew what she was doing and worked on what areas that needed very well, when I explained where they were. Again, the best I've had yet on Oahu.
As far as Thai Massage goes, I go to Thailand almost every year for my $10, 2 hr."Gumby" therapy. Not much comes close.
Others:
KAT at CC. Very professional, more stretching than anything but good.
Pinky at CC. Very good also, will hurt you. And I like it hard (sickos. Hard massage) works out those kinks.
Place in Salt lake / Stadium mall. The girl there at the time moved to the place in PC West Gate Center. OK, not impressed. A little big for back walking, not very stimulating to look at.
I like to try different places just to see, but if you want a for sure pain reducing Thai massage, I'd hit up the first one.

Also known as Angel Thai Massage according to their door. Nice place in a modified nicely furnished 1-bedroom unit.
Repeated once again, got a legit uncovered massage. No play but some roaming allowed. Sorry I forget her name but she's kind of young and pretty (even without makeup) with a nice slim body. Very friendly Thai girl.
She did a pretty thorough job and my muscles felt better afterwards.
She basically works alone but there is a second younger girl who seems to come in with her customer in hand, and the place can accommodate up to three customers.
Sessions usually go long by about 20 minutes, unless there's a customer coming.
My main reason for going there are that I needed a real massage (as opposed to the token massage in the FS places), and secondarily that there was no answer to the following when I called:

I think Thai Banyan does a good job. Everyone there tries to do it the way you want. If you are not into pain, they will back off.
